Privacy and HIPPA

The physicians and employees of the Dallas Center for Pelvic Medicine strictly abide by the laws and regulations set forth in the Health Insurance Portability and Privacy Act (HIPPA). All patients are required to sign a HIPPA form and designate those individuals approved to receive patient health information. We will not release information to those not on the form.

Appointments

We are aware that even the most organized individual will occasionally have issues arriving at the designated time for a physician appointment. We also struggle within unforeseen emergencies that interfere with our schedule. We will strive to see you as close to the appointed time as possible and will continue to update you if there are anticipated delays. We would ask that you make us aware of significant delays in arriving to the office. It may, in either case, be more reasonable to reschedule your appointment.

Co-pays and Co-insurance

Essentially every insurance policy requires a co-pay or some form of deductible payment. We would ask you to be prepared to make those payments at the time of service.
