What To Bring to Your First Appointment
New patients should have their insurance card, driver’s license, and completed patient forms. Other helpful items include a current list of medications (with dosage and frequency), medical and surgical history, drug allergies, and any recent labs or tests. If you wish to have records from another office sent to our office, you must contact that physician’s office prior to your appointment.
Returning patients should have their insurance card and driver’s license. If there have been any changes in your medical history, please be sure to have the information so we can update your medical record. If you had any labs or tests performed that were not ordered by our office, please be sure to have a copy sent to our office or bring a copy with you.
